What Is Valacyclovir?

Valacyclovir is an FDA-approved medication for herpes infections. It is an antiviral medication that helps to slow the growth and spread of certain viruses in the body, such as those that cause herpes. By interfering with the virus's ability to multiply, valacyclovir helps to manage viral outbreaks and reduce their severity and frequency.

Beyond its approved uses, valacyclovir is currently being investigated in clinical trials for a range of conditions. These include various herpes-related infections such as Herpes Simplex (14 trials), Herpes Zoster (shingles, 10 trials), Genital Herpes (7 trials), and Herpes Simplex 2 (3 trials). It is also being studied for its potential role in managing HIV Infections (13 trials) and as part of combination therapies for other serious conditions like Prostate Cancer (3 trials). Additionally, some trials involve healthy participants to assess the drug's safety and how it is processed by the body.

Uses and Conditions Under Study

Valacyclovir is extensively studied for various viral infections, particularly those caused by the herpes virus family. Conditions such as Herpes Simplex (14 trials), Genital Herpes (7 trials), Herpes Genitalis (5 trials), and Herpes Simplex 2 (3 trials) involve outbreaks of sores or blisters, often around the mouth or genital area. Valacyclovir, as an antiviral, aims to reduce the frequency and severity of these outbreaks. Similarly, it is investigated for Herpes Zoster (10 trials) and Shingles (5 trials), which are caused by the varicella-zoster virus, another member of the herpes family, leading to painful rashes.

The medication is also a focus in clinical trials involving HIV Infections (13 trials) and HIV Infection (5 trials). In these studies, valacyclovir's potential effects on viral load or its role in managing co-infections in patients with HIV are being explored.

Beyond viral infections, valacyclovir is being studied as part of combination therapies for certain cancers, specifically Prostate Cancer (3 trials). In these instances, it is often used alongside other treatments, such as gene therapies, to potentially enhance their effects or manage related viral components.

Additionally, 9 trials have included healthy participants. These studies typically focus on understanding how valacyclovir is absorbed, distributed, metabolized, and excreted by the body, as well as assessing its overall safety profile in individuals without the target conditions.

Dosing

Valacyclovir has been studied in various dosage forms, including oral capsules, tablets, and oral solutions. Dosing regimens can vary significantly depending on the condition being treated and whether it is for adults or pediatric patients.

For adults, a standard dose studied is 1000 mg taken orally twice daily. Other adult regimens have included doses of 500 mg, 750 mg, 1500 mg, 2000 mg (2 grams), and even 3000 mg (3 grams) for specific treatment protocols. For example, some studies describe a regimen starting with a 2 gram immediate dose, followed by 2 grams 12 hours later, and then 1 gram at 24 and 36 hours post-treatment. Another regimen involved 2 grams of Valtrex immediately, followed by 500 mg twice daily for three days.

For pediatric patients, dosing is typically weight-based at 20 mg/kg/dose twice daily, with a maximum dose of 1000 mg/dose. Dosage adjustments may also be made based on renal function, following established guidelines. The specific dosage and duration of treatment are determined by the medical condition and individual patient needs.

Side Effects

The most commonly reported side effect in patients taking Valacyclovir was nausea. In clinical trials, 12.0% of patients receiving Valacyclovir experienced nausea, compared to 6.9% of those on placebo. Headache was also common, affecting 11.8% of Valacyclovir patients versus 7.4% on placebo.

Other side effects reported more frequently with Valacyclovir than with placebo included:

Anxiety: 12.5% of patients on Valacyclovir experienced anxiety, compared to 6.4% on placebo (from one trial with 40 patients).
Hypoaesthesia (reduced sense of touch or sensation): 10.0% on Valacyclovir versus 2.1% on placebo (from one trial with 40 patients).
Dyspepsia (indigestion): 10.0% on Valacyclovir versus 2.1% on placebo (from one trial with 40 patients).
Urinary tract infection: 7.8% on Valacyclovir versus 5.5% on placebo.

Some side effects occurred at similar rates in both Valacyclovir and placebo groups. For example, insomnia was reported by 12.5% of Valacyclovir patients and 12.8% of placebo patients, and fatigue affected 10.1% of Valacyclovir patients compared to 10.5% on placebo.

Clinical Trial Results

Clinical trials have evaluated Valacyclovir for various conditions, demonstrating its efficacy in treating viral infections and its potential impact on neurological and cognitive function.

Herpes Simplex Encephalitis (HSE)

In a long-term study (NCT00031486) investigating the treatment of Herpes Simplex Encephalitis (HSE) with Valacyclovir, a significant difference was observed in neuropsychological outcomes. At 12 months after starting treatment, 84.8% of patients on Valacyclovir showed no or mild neuropsychological impairment as measured by the Mattis Dementia Rating Scale (MDRS), compared to only 5.6% of patients on placebo.

Genital Herpes (HSV-2)

For individuals newly diagnosed with HSV-2, a study (NCT00306293) showed that Valacyclovir significantly reduced viral shedding. Patients taking Valacyclovir 1 gram once daily experienced a mean of 2.9% days of total shedding (clinical and subclinical), a substantial reduction compared to 13.5% of days for those on placebo. Clinical shedding (presence of genital lesions) was also reduced to 0.6% of days with Valacyclovir, versus 2.4% with placebo. Furthermore, the percentage of participants with at least one genital herpes recurrence was significantly lower in the Valacyclovir group (21%) compared to the placebo group (48%).

Another study (NCT00306787) compared Valacyclovir to famciclovir for recurrent genital herpes. Both treatments showed similar effectiveness in the median time to healing of all genital herpes lesions, with Valacyclovir achieving healing in 3.01 days compared to 3.07 days for famciclovir. A separate trial (NCT00362297) found that once-daily Valacyclovir had a similar suppressive effect on HSV-2 total shedding (4% of swabs with HSV detected) as high-dose acyclovir (4% of swabs with HSV detected).

Bipolar Mood Symptoms and Cognitive Problems

In a research study (NCT00428298) exploring bipolar mood symptoms and cognitive problems, Valacyclovir demonstrated a greater improvement in neuropsychological status. Patients receiving Valacyclovir showed a 10.1% change from baseline in the Repeatable Battery for the Assessment of Neuropsychological Status at 16 weeks, compared to a 3.6% change in the placebo group.

Currently Recruiting Trials

Valacyclovir, an antiviral medication, is currently being investigated in several clinical trials for a range of conditions, from infectious diseases to various cancers. These studies aim to understand its safety and effectiveness, often in combination with other treatments. One Phase 1 trial, NCT06914479, is testing a virus-based gene therapy (AdV-HSV1-TK and AdV-Flt3L) alongside valacyclovir for pediatric and young adult patients with resectable, recurrent primary malignant brain tumors. This study, sponsored by the University of Michigan Rogel Cancer Center, seeks to determine the safety, side effects, and optimal dose of this combination in patients whose brain tumors have returned after initial improvement, with an enrollment target of 11 participants. Another study, NCT07332000, is a Phase 2a investigation into biomarkers and biodistribution of aglatimagene besadenovec plus valacyclovir for men with localized, intermediate-risk prostate cancer who are scheduled for external beam radiation therapy. This trial aims to enroll 45 men. For kidney transplant recipients, a prospective randomized pilot study, NCT07294547, is comparing valacyclovir to valganciclovir for preventing cytomegalovirus (CMV) and Epstein-Barr virus (EBV) infections. This study, led by National Taiwan University Hospital, is designed to see if valacyclovir is non-inferior to valganciclovir and plans to enroll 80 participants. Pregnant women with CMV-infected fetuses may be eligible for a Phase 3 trial, NCT05446571, comparing letermovir to valacyclovir to inhibit fetal CMV replication, with a goal of 46 participants. Valacyclovir is also being studied for its potential in pain management. A Phase 2 trial, NCT05266040, from the University of California, San Francisco, is investigating valacyclovir for acute apical abscesses, focusing on preoperative and postoperative pain, and clinical healing, with an enrollment target of 60 patients for this specific phase. Furthermore, a Phase 4 study, NCT05550194, at Vanderbilt University Medical Center, is exploring the role of Varicella zoster virus (VZV) in the enteric nervous system and its connection to achalasia, with valacyclovir as a treatment. This study aims to enroll 40 individuals. Patients receiving radiotherapy for head and neck cancer who develop oral mucositis may consider a Phase 2 trial, NCT07363031, which is evaluating valacyclovir for this painful condition, targeting 84 participants. Finally, a Phase 1/Phase 2 trial, NCT01913106, is combining HSV-tk gene therapy and valacyclovir with brachytherapy for recurrent prostate cancer, aiming to enroll 25 patients.
