What Is Relugolix?

Relugolix is an FDA-approved medication for prostate cancer. It functions as a gonadotropin-releasing hormone (GnRH) antagonist. This mechanism involves blocking the release of specific hormones from the pituitary gland, which in turn reduces the production of testosterone. By lowering testosterone levels, Relugolix acts as an androgen deprivation therapy (ADT), which can help slow the growth of hormone-sensitive cancers.

In addition to its approved use, Relugolix is currently being investigated in clinical trials for a variety of other conditions. These include prostate adenocarcinoma, different stages of prostate cancer, uterine fibroids, heavy menstrual bleeding, endometriosis, biochemically recurrent prostate carcinoma, and pregnancy complications. A total of 53 clinical trials have studied Relugolix, enrolling over 19,403 participants. The first trial began in 2014, and research continues with 21 trials currently recruiting new participants.

Uses and Conditions Under Study

Relugolix is extensively studied for various forms of prostate cancer. This includes prostate adenocarcinoma, different stages of prostate cancer (such as Stage III and Stage IIC), and biochemically recurrent prostate carcinoma. Prostate cancer is often hormone-sensitive, meaning its growth can be influenced by testosterone. As an androgen deprivation therapy (ADT), Relugolix works by lowering testosterone levels, which can help manage the disease. A total of 34 trials are investigating Relugolix for these prostate cancer-related conditions, with sponsors including Myovant Sciences GmbH and Sumitomo Pharma Switzerland GmbH.

The medication is also being explored for conditions affecting women's reproductive health. Uterine fibroids, which can lead to heavy menstrual bleeding, are the focus of 12 trials. Relugolix, sometimes used as a combination therapy (e.g., with estradiol and norethindrone acetate), may help by reducing estrogen levels, which can shrink fibroids and lessen bleeding. Similarly, endometriosis, a painful condition where tissue similar to the uterine lining grows outside the uterus, is being studied in 4 trials. By reducing estrogen, Relugolix may help alleviate the symptoms associated with endometriosis, as seen in studies like NCT04860124.

Additionally, Relugolix is being investigated in 2 trials for pregnancy complications.

Dosing

Relugolix is primarily administered as an oral tablet. It is also studied in various combination therapies, often as a single pill containing multiple active ingredients.

Different strengths of Relugolix have been investigated in clinical trials. These include 40 mg, 80 mg, and 120 mg. For prostate cancer, a dose of 120 mg daily for extended periods, such as 12 months, has been studied. This may be given as a standalone treatment or in combination with other agents like darolutamide, abiraterone acetate/prednisone, or apalutamide.

For conditions such as uterine fibroids and endometriosis, Relugolix is often studied as a combination therapy. One common formulation includes 40 mg of Relugolix along with 1 mg of estradiol and 0.5 mg of norethindrone acetate (Relugolix Combination therapy), typically taken daily. Some studies also explore a single 120 mg dose to assess its pharmacokinetics.

Side Effects

In clinical trials involving patients taking Relugolix, the most commonly reported side effect was hot flush.

Other side effects, such as headache (9.6% on Relugolix vs 10.7% on placebo), nasopharyngitis (6.7% on Relugolix vs 7.6% on placebo), hypertension (3.1% on Relugolix vs 3.8% on placebo), depressed mood (4.2% on Relugolix vs 5.5% on placebo), and upper respiratory tract infection (3.0% on Relugolix vs 4.3% on placebo), were reported at similar or lower rates in patients taking Relugolix compared to those on placebo.

Clinical Trial Results

Prostate Cancer

In studies of men with advanced prostate cancer, Relugolix demonstrated rapid and sustained testosterone suppression. In a Phase 3 study (NCT03085095), Relugolix achieved a castration rate (testosterone levels below 50 ng/dL) in 56.04% of patients by Day 4, compared to 0.00% for leuprolide acetate. By Day 15 (Week 3 Day 1), 98.71% of Relugolix patients achieved castration, versus 12.05% for leuprolide acetate. The sustained castration rate over the study period was 96.7% for Relugolix, compared to 88.8% for leuprolide acetate.

Relugolix also led to a faster and higher prostate-specific antigen (PSA) response. In the same study, the confirmed PSA response rate was 79.4% for Relugolix patients, compared to 19.8% for leuprolide acetate. For patients who discontinued treatment, testosterone recovery was significantly higher with Relugolix, with 53.93% to 93.01% of patients recovering testosterone levels, compared to 3.23% to 10.12% for leuprolide acetate. Additionally, Relugolix was associated with a lower rate of major adverse cardiovascular events (MACE) at 2.9%, compared to 6.2% for leuprolide acetate.

Another Phase 2 study (NCT02083185) further supported the rapid action of Relugolix, showing that patients taking either Relugolix 120 mg or Relugolix 80 mg achieved testosterone concentrations below 50 ng
