What Is Glofitamab?

Glofitamab is a medication that has been approved for marketing in China for the treatment of patients with relapsed/refractory diffuse large B-cell lymphoma (R/R DLBCL). It is a cytolytic monoclonal antibody, meaning it is an antibody designed to target and kill specific cells. Glofitamab works by targeting the CD19 protein found on the surface of certain cancer cells, particularly B-cells involved in lymphomas. By binding to CD19, it helps the immune system identify and destroy these cancerous cells.

Globally, Glofitamab is currently being investigated in clinical trials for various types of lymphoma. As of the latest data, there are 74 trials involving Glofitamab, with 39 trials actively recruiting new participants. These studies have enrolled a total of 7,749 participants, with the earliest trial starting on March 9, 2017, and the latest projected to conclude on April 14, 2026.

Uses and Conditions Under Study

Glofitamab is being studied for its potential to treat several types of lymphomas, which are cancers that begin in infection-fighting cells of the immune system called lymphocytes. The drug's mechanism of targeting CD19-expressing B-cells makes it relevant for a range of B-cell lymphomas.

Dosing

Glofitamab is administered as an intravenous infusion, with specific dosing regimens being studied in clinical trials. A common approach involves a "step-up" dosing strategy to help manage potential side effects. For example, one regimen starts with a dose of 2.5 mg on Cycle 1 Day 1, followed by 10 mg on Cycle 1 Day 8. This is then typically followed by a maintenance dose of 30 mg administered every 3 weeks for a total of 3 cycles.

For patients who achieve a partial response (PR) or better after these initial cycles, further treatment may include autologous stem cell transplant (ASCT) followed by an additional 3 cycles of post-consolidation Glofitamab at 30 mg every 3 weeks. Glofitamab is being studied both as a monotherapy and in various combination therapies. These combinations include other agents like polatuzumab vedotin, R-CHP (rituximab, cyclophosphamide, doxorubicin, prednisone), obinutuzumab, pirtobrutinib, and CAR T-cell therapy, among others, to explore enhanced efficacy for conditions such as relapsed/refractory Mantle Cell Lymphoma and Diffuse Large B-cell Lymphoma.

Side Effects

In a study (NCT04313608) involving patients with relapsed or refractory diffuse large B-cell lymphoma and high-grade large B-cell lymphoma, the safety of Glofitamab in combination with gemcitabine and oxaliplatin (GemOx) was compared to mosunetuzumab with GemOx. The study did not include a placebo group. The most common side effects reported were:

Clinical Trial Results

In a clinical study (NCT04313608) evaluating patients with relapsed or refractory diffuse large B-cell lymphoma and high-grade large B-cell lymphoma, the effectiveness of Glofitamab in combination with gemcitabine and oxaliplatin (GemOx) was compared to mosunetuzumab with GemOx.

The complete response rate, which signifies the disappearance of all signs of cancer based on PET/CT scans, was 23.5% for patients treated with Glofitamab plus GemOx. In comparison, 50.0% of patients treated with mosunetuzumab plus GemOx achieved a complete response.

The objective response rate (ORR), which includes both complete and partial responses to treatment, was 35.3% for the Glofitamab combination. For the mosunetuzumab combination, the ORR was 50.0%.

Currently Recruiting Trials

Researchers are actively recruiting participants for clinical trials investigating glofitamab across various B-cell lymphomas. These studies aim to understand the drug's safety and effectiveness, often in combination with other therapies, for patients whose disease has returned or not responded to previous treatments. One ongoing Phase 2 trial, NCT07003295, is testing glofitamab alongside obinutuzumab in patients with mantle cell lymphoma (MCL) that has relapsed or is refractory after CAR-T cell therapy. This study, sponsored by the National Cancer Institute (NCI), aims to enroll 20 participants. Another Phase 2 study, NCT07257055, sponsored by M.D. Anderson Cancer Center, is exploring glofitamab consolidation after BTKi-rituximab induction for 30 patients with high-risk untreated MCL. For patients with relapsed or refractory large B-cell lymphoma (LBCL) with specific genetic changes (TP53 aberrations), a Phase 2 study (NCT07240194) is evaluating glofitamab in combination with a PD-1 inhibitor and lenalidomide, targeting 24 participants. Central nervous system (CNS) lymphoma is the focus of a Phase 1 trial (NCT06922604) at City of Hope Medical Center, which is assessing glofitamab with obinutuzumab in 20 patients. Hoffmann-La Roche is sponsoring a Phase 1 study (NCT06624085) in the U.S. to evaluate glofitamab combined with gemcitabine and oxaliplatin for 50 participants with relapsed or refractory diffuse large B-cell lymphoma (DLBCL). A larger Phase 3 trial, NCT06084936, is comparing glofitamab monotherapy against an investigator's choice of treatment for 182 patients with relapsed or refractory MCL. Additionally, a Phase 1/2 study (NCT05533775) is investigating glofitamab for 65 pediatric and young adult participants with relapsed or refractory mature B-cell non-Hodgkin lymphoma.

Where to Participate

Clinical trials for glofitamab are widely accessible, with studies currently active across 156 sites in 95 cities and 39 states. This broad reach ensures that many patients may have the opportunity to participate. The top cities with the most recruiting sites include: Eligibility for these trials generally includes individuals aged 6 to 80 years, of all genders. Healthy volunteers are not eligible to participate, but children are included in some studies.

Development Timeline

The journey of glofitamab began with its first clinical trial on March 9, 2017. Initially, early explorations included conditions such as IBS-C and hyperphosphatemia, before the development path significantly broadened to focus on B-cell malignancies. Hoffmann-La Roche has been a primary driver of glofitamab's development, sponsoring 17 of the 74 total trials conducted to date. Over time, the pipeline expanded to address a wide range of B-cell lymphomas, including Large B-cell Lymphoma, Diffuse Large B-Cell Lymphoma (DLBCL), Mantle Cell Lymphoma (MCL), and various forms of Non-Hodgkin Lymphoma. The drug's development has progressed through multiple phases, with 40 trials reaching Phase 2, 14 in Phase 1, and 10 as Phase 1/Phase 2 studies. Three trials are currently in Phase 3, representing advanced stages of investigation. These studies have collectively enrolled 7,749 participants, with the latest trial projected to conclude on April 14, 2026.
