Job Description

Job Responsibilities: Provide comprehensive outpatient primary care services to adult patients (and pediatric patients if Family Medicine trained)
Diagnose, treat, and manage acute and chronic medical conditions
Perform routine physical exams, wellness visits, and preventive screenings
Develop and monitor individualized treatment and care plans
Prescribe medications and interpret diagnostic test results
Coordinate care with specialists, behavioral health, and support staff
Document patient encounters accurately in EMR systems
Educate patients on disease prevention, lifestyle modification, and treatment adherence
Participate in quality improvement and population health initiatives
Maintain compliance with clinical, regulatory, and organizational standards
Qualifications: MD or DO from an accredited medical school
Completion of an accredited Family Medicine or Internal Medicine residency program
Board Certified in Family Medicine or Internal Medicine
Active or eligible Indiana medical license
Active DEA license or ability to obtain prior to start
Minimum 1–2 years of outpatient clinical experience preferred
Experience in FQHC, Community Health Center, or PCMH settings preferred
Strong clinical, communication, and patient-centered care skills
Proficient in EMR systems (eClinicalWorks preferred)
Ability to work collaboratively in a multidisciplinary care team
Working Hours: Working Hours:
Monday: 8:00 AM – 5:00 PM
Tuesday: 8:00 AM – 5:30 PM
Wednesday: 8:00 AM – 5:00 PM
Thursday: 8:00 AM – 5:30 PM
Friday: 8:00 AM – 3:00 PM
