Practice Info
• This is a field-based role requiring travel to various clinical sites and patient locations across our Southeast LA service areas.
• Service Areas include, but are not limited to: Southeast LA: Paramount, Downey, Norwalk, Huntington Park, Bellflower
Responsibilities
• Provides a range of primary, acute and/or specialty healthcare services for patients in home or when appropriate virtual settings.
• In states that do not require collaborative agreements, functions as the primary care provider.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
• Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices.
• May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team.
• Formulates and documents care plan.
• Prescribes medication or other forms of treatment.
• Might participate in On-Call program, which may include after-hours, weekend and holiday calls and visitation
Compensation
• This position is bonus eligible based on individual and company performance.
• $119,313 to $178,968
Requirements
• Requires an MS in Nursing
• Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
• Requires a current, active, valid and unrestricted RN license and NP license in Nevada.
• Satisfactory completion of a Tuberculosis test is a requirement for this position.
